My child takes part in Aunger Elite Football sessions voluntarily. Football is physical and carries a risk of injury.
To the extent permitted by law, I release Aunger Elite Football and its coaches from liability for injury, loss or damage arising from participation.
My child is in good health and fit to take part, and I have listed any relevant medical conditions above.
In an emergency, if I can't be reached, I authorise the coach to arrange reasonable first aid or medical treatment.
