In consideration of being permitted to participate in coaching, training and activities provided by Aunger Elite Football ("the Academy"), I acknowledge and agree to the following:
Assumption of risk. Football and physical training carry an inherent risk of injury. I voluntarily accept these risks on behalf of the player named above.
Health & fitness. The player is in good health and physically capable of participating, and any relevant medical conditions have been disclosed above.
Medical treatment. If the player is injured or unwell and I cannot be contacted, I authorise the Academy to arrange appropriate first aid or medical treatment.
Release. To the extent permitted by law, I agree not to hold the Academy or its coaches liable for injury, loss or damage arising from ordinary participation, except where caused by the Academy's negligence. Nothing in this waiver limits any rights that cannot be excluded under the Australian Consumer Law.
Conduct. The player will follow the reasonable instructions of coaching staff and behave respectfully. The Academy may suspend participation for unsafe or disruptive behaviour.
