By signing below, I confirm that I am the parent or legal guardian of the player named above and I consent to their participation in football clinics run by Aunger Elite Football ("the Academy").
Assumption of risk. I understand that football involves physical activity and carries an inherent risk of injury. I acknowledge these risks and agree that my child participates voluntarily.
Health & fitness. I confirm my child is in good health and physically able to take part, and I have disclosed any relevant medical conditions above.
Medical treatment. In the event of injury or illness where I cannot be contacted, I authorise the Academy to arrange appropriate first aid or medical treatment for my child.
Code of conduct. I agree that my child will follow the reasonable instructions of the coaching staff and behave respectfully toward coaches and other players. The Academy may remove a participant for unsafe or disruptive behaviour.
Payment & cancellations. A place is confirmed once registration is complete. Please give reasonable notice of cancellation. Sessions may be rescheduled due to weather or unforeseen circumstances.
